American Citizen Services Day: Düsseldorf Sept 22

DATE: Wednesday, Sept 22, 2010 from 10:00 to 14:00

LOCATION: U.S. Consulate General Düsseldorf, Willi-Becker-Allee 10, located on Bertha-von-Suttner Platz behind the Düsseldorf Hauptbahnhof.

(Please leave ALL ELECTRONIC DEVICES at home, in your vehicle, or in storage at the train station. There will be ONLY LIMITED space at the Consulate to store them.)

SERVICES: Consular Officers will accept reports of birth abroad (CRBAs) and passport applications and will perform notarial services (for American citizens or for use in the United States ONLY).

CHECKLIST OF "MUST BRING WITH YOU ITEMS" FOR PASSPORT APPLICATIONS AND CRBAs:

  • Original "raised or multi-color seal" support document and copy of any citizenship documents, including (any applicable documents from the list):
birth certificate,
naturalization certificate,
certificate of citizenship, marriage certificate,
divorce decree,
adoption decree,
legal name change document, etc.
  • existing passport (if applicable) and a copy of the name/picture page
  • two 2x2 in or 5x5 cm identical passport pictures that are not older than 6 months.
  • payment can be cash (euros or dollars) or credit card (american express, mastercard or visa; no ec cards) for passport services but notarial services must be paid in cash.

- Initial Adult Passport Book: $135
- Adult Passport Book Renewal: $110
- Minor (under the age of 16) Passport Book: $105
- Adult Passport Card: $30
- Adult Initial Applicant applying for a Passport Card ONLY: $55
- Minor Passport Card (In connection with a book application): $15
- Minor Passport Card ONLY: $40
- Consular Report of Birth Abroad: $100
- Additional Visa Pages: $82 (NOTE: This is a new fee)
- Issuance of a Certificate of Loss of Nationality based on a formal Renunciation of U.S. citizenship: $450 (NOTE: This is also a new fee)

  • ds-11 for 1st time after turning age 16, mutilated, lost/stolen passport: must be completed on-line and printed out. for children under 16, both parents must be present.
  • ds-82 for 2nd or any subsequent passport after turning age 16: must be completed on-line and printed out

Checklist of "must bring with you items" for notarials:

  • in addition to the documents that need notarized, bring proof of identification.
  • the fee is $ 30 or euro equivalent for the first notarization on a given day and $ 20 or euro equivalent for each subsequent notarization dealing with the same subject or transaction on the same day.
